I will notify CCI of any changes to my address, phone number, or email so they can update their records. CCI will not share my information with any other organization without my permission, except as permitted.

By electronic signature, I authorize CCI to contact agencies and veterinary offices to confirm the responses I've provided, and to share information about my residence and any pets in my care (past or present, including pets I'm caring for on behalf of someone else). Most rental agents and veterinary offices require this screening form before releasing information.

This Screening Form is a multi-page document that becomes part of the Adoption Contract. CCI permits cats to go home the same day of adoption but relies on the truthfulness of these answers. Falsification of any information voids the contract; CCI may then arrange the return of the cat. CCI may decline an adoption for any reason without stating it, or may reclaim the cat at any time if information here is incorrect or the animal is not being properly cared for. The adoption fee will be forfeited in those cases.

CCI may use all legal remedies available to facilitate a return; if legal fees and costs are incurred, you may be responsible for them. This Screening Form is the property of CCI and will not be returned to you.

Candy's Cats, Inc. was incorporated in 2005 as a 501(c)(3) nonprofit, no-kill cat rescue, licensed in the State of Florida to receive charitable donations.

Disclaimer: Donations are made to CCI. CCI reserves the right to use donations in the best interests of the animals in our care. If you are donating to support a particular fundraising effort and the funds raised exceed that effort's goal, the excess funds will be deposited into the general operating fund for use in helping all animals in the care of CCI as CCI deems appropriate. All monies donated go to the vetting, medicine, spay/neuter, food, litter, operations, and supplies necessary to care for the animals and to administer CCI as a 501(c)(3). All donations are tax-deductible.
